Observation Students

Student Program Requirements for Observation (Job Shadow) Experiences Without Direct Patient Care

All of the required documentation must be submitted in one complete packet sent directly from the affiliated school or student to studentprograms@carterethealth.org TWO weeks prior to the rotation start date.

Everyone observing with Carteret Health Care must submit the Student Information Sheet and Photography Release.

Observation Over 24 Hours Requirements

In addition to the above forms, the following documents are also required for anyone observing over 24 hours:

-Item #1- Letter of Agreement & Certificate of Insurance. This is your school’s responsibility to ensure there is an established Educational Institutional Letter of Agreement and certificate of insurance is on file. Have your school contact studentprograms@carterethealth.org if your observation is more than 24 hours.
-Item #2- Student Information Sheet. Please save the form to your computer before including it in the packet or the form can be printed. Email studentprograms@carterethealth.org for an observation packet.
-Item #3- Documents and Module Requirements. The following documents and modules are required to complete and return.

-Item #4-Immunization Record and TB screening

  • TB PPD - TB Gold Quantiferon Test OR Two Step Tests
  • MMR - 2 doses or positive titer (results include ref. range)
  • HEP B - Energix B or Recombivax - B 3 doses (0, 1, and 6 months) OR Heplisav - B 2 doses (0 and 1 month) or Positive titer (results include ref range) (For Declination or waiver, a copy must be on file)
  • TDAP - TDAP is optional or 1 dose TDAP/ TD booster every 10 years
  • VARICELLA - 2 doses or positive titer (results include ref range)
  • INFLUENZA - Each season by 11/15
  • COVID 19 - Either 2 doses of Pfizer or Moderna brand or one dose of Johnson and Johnson brand- must be 2 weeks after the last dose to be considered fully vaccinated.

-Item #5- Background check (EXCEPT HIGH SCHOOL STUDENTS AND MINORS) The nationwide criminal background check is the student’s responsibility and expense. Most schools have established contracts or processes for students to get criminal background checks. However, there are some programs that require the student to obtain a background check independently. The background check must include: Criminal records check (CRC) includes a nationwide check in each state for the past 7 years. The check is based on the student’s security number and address history. The check also must include sex offender and OIG.
