Medical Records

To receive a copy of your medical record, print out and complete our Request for Health Information form.

Requests for Health Information forms can be faxed to 252-499-6939 or mailed to Health Information Management, Attention Release of Information, 3500 Arendell Street, Morehead City, NC 28557. You can also bring the completed form to the Medical Records Department in Carteret Health Care. If you have any questions about your medical record, please contact the Medical Records Department at 252-499-6528. The department is open for walk-in customers from 8:30am - 5:00pm, Monday through Friday.
