Susan Davis, RN Honored to Present during Accountable Care Symposium

Susan Davis, RN of Carteret Health Care has been invited to present during Caravan Health’s third annual Accountable Care Symposium as part of an expert panel on the key issues affecting the value-based care landscape. Beginning on December 5, 2018 in Phoenix, Arizona, Ms. Davis is honored to participate in the conference that will be facilitated by the industry’s thought leaders, with years of experience and strategies about the advancement of value-based payment systems.

Ms. Davis will discuss Carteret Health Care’s journey with value based programs that began in 2011. She will share Carteret’s successes, challenges and strategies in population health. The key focus areas to be shared will be annual wellness visits with standardized screens, resources to address issues, advance care planning and Transitional Care Management including care coordination support and palliative care resources. Also, Chronic Care Management for complex patients was part of the initial focus area.

With over 30 years of nursing experience, Ms. Davis is currently the Population Health Nurse at Carteret Health Care serving the Medicare ACO patients for Bayou Basin. Her past experience includes positions where she worked in state and national roles with ACOs for Medicare, Medicaid and commercial populations. With a graduate degree in public health from the University of North Carolina Chapel Hill, she is certified in case management and health coaching. Her current role providing support for practices in the ACO and working directly with patients is one of the most rewarding experiences of her career.
