If you’re scheduled for an outpatient procedure, you may be contacted by telephone to complete the pre-registration for your visit. This will allow for a quicker service on the day you present at Carteret Health Care or the Imaging Center.

In most situations, your first encounter at CHC will be with one of our Patient Access Representatives. For admission to the hospital, or for outpatient testing, please present to the Patient Access Department located inside the main entrance. The registration process consists of gathering information including address, next-of-kin and insurance information. If your physician has given you a paper order, please present it to the Patient Access Representative. The foremost concern of our staff is your safety and confidentiality. By verifying your information and picture identification at each visit, we can reduce the risk of errors and protect your identity. We look forward to serving you.
